You can make changes to a style by clicking "Edit" after selecting it in Style Manager. To change the layout of citations, click on the templates and edit the fields of the sample references as though you were formatting them in an ordinary word-processor. For example, you can make book titles print in italic by highlighting the field "Title" in the book template and setting the text style to italic.

You should save your changed style as a copy, rather than trying to over-write the existing one. Choosing File > Save As will cause a copy of the style to be saved to a personal Styles folder in Documents. This has the added advantage that later updates to the EndNote program will not over-write your custom styles.
